GRANT REPORTING

Complete the following if your organization is a recipient of a grant of $10,000 or more from the Foundation.
The Foundation requires a report be submitted at the end of the fiscal year the grant was used. This report acts as a substantive record of the activities conducted, the products produced throughout the year and how goals set forth in the proposal were met. Please click on the link below for the Grant Report Template which should be completed and sent via postal mail or e-mail (as an attachment) to the Foundation.
