Complete the following if your organization is a recipient of a grant of $10,000 or more from the Foundation.
The Foundation requires a report be submitted at the end of the fiscal year the grant was used. This report acts as a substantive record of the activities conducted, the products produced throughout the year and how goals set forth in the proposal were met.
